Key Takeaways

What you'll learn from this article

  • 1Railway doctors and railway paramedical staff follow different recruitment routes.
  • 2The regular doctor route is linked to UPSC CMS.
  • 3MBBS graduates can be eligible subject to the prescribed conditions.
  • 4Railway medical service offers a structured government career path.
  • 5Always verify the latest notice before applying.

You search forrailway medical officer recruitment 2026 and come across a page stating RRB doctor vacancy. Following this, you click on an RRB application portal.

It seems simple. However, it actually isn’t.

For an MBBS doctor, the most important distinction is that the regular Railway medical officer entry route is tied to the UPSC Combined Medical Services Examination, where the Railway post is Assistant Divisional Medical Officer (ADMO) in the Indian Railway Health Service. In the 2026 examination, UPSC notified 450 Railway ADMO vacancies.

That one distinction can save you from preparing for the wrong recruitment process.

Recruitment of Railway Medical Officers 2026: Which Way to Go?

For a regular Railway doctor appointment, think UPSC CMS first, RRB second.

The 2026 UPSC notification covered Assistant Divisional Medical Officer posts in the Railways under Category II. The application was made through the UPSC online system, not through an RRB paramedical application form. The 2026 notification opened on 11 March, applications closed on 31 March, and the written examination was conducted on 2 August 2026.

This also explains why the phrase “RRB CMS notification” can be misleading. There was a UPSC Combined Medical Services Examination notification for Railway doctor posts; that should not be confused with an RRB Centralised Employment Notice for paramedical staff.

There is another route worth knowing. Zonal Railways can separately engage Contract Medical Practitioners (CMPs) when required. That is a contractual engagement, not the same career route as becoming a regular ADMO in IRHS. Railway contractual notices also make clear that such engagement does not itself confer a right to regularisation.

Railway MBBS Job Eligibility

For CMSE 2026, UPSC required candidates to have passed the written and practical parts of the final MBBS examination. Candidates awaiting their final MBBS result could also apply provisionally, subject to the prescribed conditions. A candidate who has not completed a compulsory rotating internship could be admitted to the examination, but appointment would take place only after completing the internship.

For the 2026 examination, the general upper age limit was 32 years as on 1 August 2026, with specified relaxations for eligible categories. The Railway post was also subject to the prescribed physical and medical standards.

The examination itself had a 500-mark written component, followed by a 100-mark Personality Test for candidates qualifying the written stage. The written examination consisted of two papers covering General Medicine & Paediatrics, and Surgery, Gynaecology & Obstetrics, and Preventive & Social Medicine.

Railway MO Salary: What Can You Actually Expect?

For the regular Railway doctor the 2026 UPSC notification gives the clearest official figure: ADMO is Group A Junior Scale, Pay Level 10, with a basic pay range of ₹56,100–₹1,77,500 and Non-Practising Allowance (NPA) as applicable under the rules.

Private practice is prohibited.

Pay structure

Assistant Divisional Medical Officer

Level 10: ₹56,100–₹1,77,500 basic + applicable NPA

Contract Medical Practitioner

Remuneration depends on the specific zonal contract notification

The actual monthly earnings can vary with applicable allowances, posting, deductions and Government orders.

 Railway Doctor Career  Growth

The safest way to understand the growth is as a graded Group A medical service, rather than assuming that every doctor moves through identical titles on an automatic timetable.

The 2026 UPSC notice states that promotion prospects are governed by the applicable Railway Medical Service rules and Ministry of Railways orders.

ADMO / Junior Scale — Level 10

Senior medical grade — Level 11

Junior Administrative Grade — Level 12

Higher medical/administrative grades — subject to applicable rules

Exact promotion eligibility, residency requirements and nomenclature should always be checked against the rules applicable at the time of service.

What Does a Railway Medical Officer Actually Do?

This is not a government version of a private hospital OPD job. The UPSC notification describes ADMO responsibilities including:

  • attending indoor wards and the outpatient department,

  • conducting physical examinations of candidates and serving employees, and

  • undertaking duties connected with Railway medical services.

Officers may also be posted to Railway health units and dispensaries, including locations away from major cities.
The 2026 service conditions also matter.

Appointment includes:

  • probation,

  • training,

  • departmental requirements and the

  • possibility of transfer within the Railway system.
    Private practice is prohibited.

So ask yourself what you actually want from your career.

If you want structured government service, institutional medicine and long-term progression, this route can make sense.

If you want faster movement into pharmaceutical research, medical affairs or other non-clinical careers, that is a different decision altogether.

Frequently Asked Questions

1. Will there be RRB doctor vacancies in 2026?
In the 2026 cycle, the UPSC Combined Medical Services Examination was the regular route for Railway doctor recruitment with 450 openings for Assistant Divisional Medical Officers in Railways. RRB does recruitment for a lot of railway categories, including paramedical posts on its own. It is not to be confused with the usual ADMO entry process for MBBS doctors.
2. Does UPSC recruit for Railroad Medical Officer?
Yes, for the regular Combined Medical Services Examination for Indian Railways Health Service entrance. he 2026 UPSC advertisement clearly mentioned the CMSE job of Assistant Divisional Medical Officer in Railways. So if the candidate has done MBBS and is looking for the RRB recruitment pages, they might be looking at the wrong place.
3. What are the qualifications for the Railroad Medical Officer?
For the 2026 CMSE, candidates were required to have passed the written and practical parts of the final MBBS examination. Students awaiting their final exams were eligible for appointment provisionally, while appointment of students who had not completed the necessary rotating internship was subject to its completion. There were physical and medical criteria.
4. What is the pay of Railroad MO?
Pay Level 10 with a minimum basic pay of ₹56100 and maximum basic pay of ₹177500, and Non-Practising Allowance as admissible under the regulations of Government for the post of Assistant Divisional Medical Officer recruited through the 2026 UPSC CMS route, has been mentioned in the Official Notification. An aggregator’s single “in-hand salary” figure can be misleading, as actual monthly take-home salary would depend on appropriate allowances and deductions.
5. Can an MBBS doctor apply without doing his internship?
As per the UPSC notification 2026, an MBBS applicant who has not yet completed the mandated rotational internship can be educationally eligible for the exam. But that doesn’t mean an appointment is immediate. The UPSC has explicitly stated that the appointment will be subject to completion of the required rotational internship and fulfilment of applicable conditions.
6. Railroad doctor Vacancy Age Limit?
The highest age limit for CMSE 2026 was 32 years on 1 August 2026. The UPSC provided relaxations for certain groups such as SC, ST, OBC, eligible defence personnel, ex-servicemen and several categories of PwBD. The cut-off date and rules can change, so always review the notification for the current year.
7. Is there any distinct RRB CMS notification for Doctors?
Terminology is complicated. Railroad doctor jobs 2026 were notified through the UPSC Combined Medical Services Examination and not a separate RRB CMS examination. RRB recruitment notifications may have paramedical or other railroad recruitment positions. But an MBBS applicant who wants to follow the normal ADMO route has to stick to the UPSC CMSE notifications.
8. Do railroad doctors have private practice?
The official notice of UPSC 2026 mentions that the post of Assistant Divisional Medical Officer is entitled to Non-Practising Allowance and Private Practice is not allowed. The doctor who is thinking of taking on the job should therefore regard it as a full-time career in Government medicine, rather than assume that private consultation work can go on at the same time.
9. Is the Contract Medical Practitioner employment similar to normal Railroad Medical Officer jobs?
No. Zonal Railroads can engage doctors as Contract Medical Practitioners, but these are contractual appointments with their own terms and conditions. The contractual notices of the Railroads provide that such service does not immediately give the right of regular appointment or absorption. The ADMO standard route is a Group A service route in its own right.
10. Where to look next for Railroad doctor vacancy?
Start with the official examination & recruiting pages of UPSC for CMSE related Railroad doctor recruitment. Use the official Railroad websites for zonal contractual medical practitioner notices. Do not depend solely on a site named "RRB Doctor Vacancy 2026". Jobslly can also be handy when you are exploring healthcare vacancies outside Government recruitment especially clinical and non-clinical roles across India.
