 Job Summary 

This position is for a Junior Clinical Pharmacist on a fixed-term contract at Hinduja Hospital, Mahim. The person will check prescriptions, assist with medication reconciliation and work, on drug safety projects under guidance. The job also includes counselling, providing drug information, and taking part in antimicrobial stewardship activities. Applicants should have 1 to 3 years of experience and a Pharm D qualification is preferred.

 Key Responsibilities 

Clinical Pharmacy Services

  • Review prescriptions for the right dose, how often to take it the way it is taken any interactions with medicines if it is repeated by mistake and if it is the right choice while under supervision.

  • Help with checking medicines when a patient comes in moves between departments, or leaves the hospital.

  • Do audits of prescriptions and summaries given when a patient is discharged.

  • Find out any changes or advice given by the pharmacy team.

  • Help put hospital treatment plans and rules about medicines into action.

  • Keep track of data, about the activities done by the pharmacy team.

Medication Safety and Pharmacovigilance

  • Identify and report adverse drug reactions.

  • Report medication errors and near-miss incidents.

  • Assist in monitoring high alert and LASA medications.

  • Maintain medication safety records and reports.

Patient Counseling and Education

  • Counsel patients on medication use, storage, side effects and adherence.

  • Provide discharge medication counselling.

  • Assist in preparing patient education materials.

  • Promote safe medication practices among patients and caregivers.

Drug Information Services

  • Assist in answering to drug information queries.

  • Review scientific literature, guidelines, and drug databases.

  • Support preparation of drug information documents and presentations.

Antimicrobial Stewardship Program

  • Participate in antimicrobial stewardship rounds.

  • Aid in antibiotic utilization reviews and data collection.

  • Support monitoring of antibiotic appropriateness and compliance.

  • Assist in ICMR and hospital antimicrobial stewardship projects.

Quality and Accreditation

  • Support NABH compliance and medication management standards.

  • Participate in departmental audits and quality improvement activities.

 Educational Qualification 

  • A Pharm.D (Doctor of Pharmacy) qualification is preferred for this role.

  • Candidates should have around 1 to 3 years of experience in clinical pharmacy.

 Skills Required 

  • Good knowledge of pharmacotherapy.

  • Strong skills when it comes to reviewing prescriptions and making sure medications are correctly recorded.

  • Ability to talk clearly with patients and explain things well.

  • Knowledge about keeping medications safe and reporting any issues.

  • Understanding of NABH standards.

  • Good at looking up information from research and providing details about drugs.

  • Strong ability to write records and reports.
